Position SummaryThe PRN 7 PM - 7 AM Hospice Continuous Care Licensed Practical Nurse (LPN) provides skilled nursing care and compassionate support to hospice patients experiencing acute symptoms requiring continuous bedside care. The LPN collaborates closely with the RN Case Manager, interdisciplinary team, and family caregivers to ensure comfort, safety, and symptom management during crisis periods in accordance with the hospice plan of care and regulatory guidelines.
Essential Duties and Responsibilities- Provide direct, continuous bedside nursing care to hospice patients in the home, assisted living, or nursing facility settings.
- Monitor and manage symptoms such as pain, respiratory distress, agitation, or other end-of-life concerns under the supervision of the RN Case Manager.
- Administer medications, treatments, and interventions as prescribed in the plan of care.
- Observe, document, and report changes in the patient’s condition promptly to the supervising RN or physician.
- Maintain accurate, timely, and complete clinical records per hospice and regulatory standards.
- Educate and support patients, caregivers, and family members on comfort measures and end-of-life care needs.
- Collaborate with interdisciplinary team members—including social workers, chaplains, aides, and volunteers—to ensure coordinated care.
- Participate in team meetings, in-service education, and quality improvement initiatives.
- Demonstrate empathy, professionalism, and adherence to hospice philosophy and standards of care.
- Current and active LPN/LVN license in the state of Georgia.
- Minimum of one (1) year nursing experience preferred; hospice, palliative, or home health experience strongly desired.
- Demonstrated ability to provide compassionate, patient-centered care.
- Excellent assessment, communication, and documentation skills.
- Reliable transportation and valid driver’s license with auto insurance.
- Flexibility to work overnight, weekend, or extended shifts as needed for continuous care coverage.
- Frequent standing, walking, and lifting of patients or equipment (up to 50 lbs).
- Ability to perform tasks requiring good manual dexterity and visual acuity.
- Ability to travel between patient homes and facilities.
