The LVN provides direct nursing care to an assigned group of patients, under the appropriate supervision of a Provider, Registered Nurse, by applying clinical nursing knowledge, skills, and ensuring the safety and comfort of patients and families according to legal, organizational and professional standards. The LVN prioritizes and makes judgments concerning basic needs of multiple patients to organize care. Assists with outreach initiatives and goals set forth by the Primary Care at Home leadership.

Ongoing liaison between patient, multispecialty care departments and approved Facilities.

• Graduate of an accredited LVN Program
• License Vocational Nurse in the State of Texas
• Current BCLS
• 3+ years of clinical LVN experience in an outpatient or acute care setting
• Ability to travel throughout the greater Houston area to facilities and patient homes
• Driver's license and access to reliable personal transportation and current insurance coverage

• 1+ years of Home Health experience
• Care Management / Disease Management experience
• EPIC experience
• Windows based computer skills

The hourly pay for this role will range from $23.41 to $41.83 per hour based on full‑time employment. We comply with all minimum wage laws as applicable.
