Position SummaryWe are seeking a skilled and compassionate Registered Nurse (RN) and Licensed Vocational Nurse (LVN) to join our hospice team on a per diem basis. The Hospice RN will provide comprehensive nursing care to patients in their homes or care facilities, focusing on pain and symptom management, patient/family education, and emotional support.
Responsibilities- Deliver direct nursing care and symptom management to hospice patients.
- Conduct patient assessments and develop individualized care plans. (If LVN, assist with development and implementation of care plans)
- Collaborate with the interdisciplinary team (IDT) to ensure coordinated care.
- Provide education and emotional support to patients, families, and caregivers.
- Document patient care in compliance with state, federal, and organizational standards.
- Participate in on‑call rotations, as needed.
- Current RN license.
- Strong communication skills.
- Compassionate, patient‑focused, and able to support families during sensitive times.
- Valid driver’s license and reliable transportation.
- Per diem position with flexible scheduling.
- Competitive hourly rate. (Starts at $100 for RN visit)
- Mileage reimbursement (beyond 30 miles from home base).
