About the Role:

As a SEND Progression Mentor, you will play a vital role in supporting young people with learning disabilities, autism, sensory needs, and physical disabilities to achieve their Education, Health and Care Plan (EHCP) outcomes.

Key roles and responsibilities:

To provide outcome-focused, strength based and time-limited interventions for people in the community, at the Southwark Resource Centre or other locations. In line with their EHCP listed outcomes.
To provide person-centred support for people with complex needs, by developing communication skills and participation/engagement in tailored activities to their maximise opportunities and increase independence.
To act as keyworker in identifying and facilitating person-centred, meaningful EHCP outcomes with the person, and their wider support networks.
To plan appropriate interventions to maximise social inclusion, wellbeing and promote independence and resilience.
To connect people with their local network, Community Voluntary Sector and their wider community to promote independence and well-being.
To identify what assets, support networks and local resources are available and accessible to persons.
To provide advice, information and connect individuals and their support networks to local community services.  To have the opportunity to have choice and control over the support they access.

Knowledge, including educational qualifications:

NVQ level 3 in Health and Social Care (or equivalent) or the willingness to undertake
Knowledge of the social, physical and emotional needs of the client group, including communication methods.
Knowledge and understanding of safeguarding procedures and experience of contributing to the safeguarding persons’ process and supporting vulnerable persons to be safe.

Experience:

Experience of working with people with disabilities either in a paid or voluntary capacity and providing tailored support.

Experience of developing outcome-focused, person-centred support planning; and assisting individuals with support planning or goal setting.

Aptitudes, Skills &

Competencies:

Literacy, IT and numeracy skills equivalent to Level 2 NVQ
Including ability to prepare reports.
Ability to communicate effectively and respectfully with persons, their families and carers, and with other professionals.
Ability to deal effectively and safely with behaviours of concern within a PBS framework.
Ability to assist people with their physical needs, such as toileting, transfers, mobility and eating, whilst ensuring that the service user is treated with dignity and respect at all times. Paying attention to a person’s sensory needs.
Ability to complete risk assessments and implement safe plans, with an awareness of Health and Safety legislation and responsibilities.
Good organisational and planning skills.  To be able to organise and prioritise own work and meet the varied demands of the role.
Ability to work as part of a team and use own initiative where appropriate.

Special Conditions of Recruitment:

The post holder will work within the terms and conditions established by the council for its staff and will be required to obtain enhanced DBS clearance.
The post holder should be physically able to meet the demands of the post in relation to supporting the personal care and moving and handling of disabled service users.
Ability to deliver a service based on individual needs of service users including the requirement for flexible working including potential evening and weekend work.
An understanding and commitment to key council policies and initiatives e.g. personalisation, managing diversity and equal opportunities and the ability to apply to work practice.
