Home Hospice Nurse - $5,000 Sign-On Bonus

Location:

Servicing patients within a 30-mile radius of Pella, IA

Schedule:

Full-Time, Rotating On-Call

• Provide skilled nursing care to terminally ill patients in their homes within a 30-mile service area.
• Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team.
• Conduct comprehensive assessments, manage pain and symptoms, and coordinate medication administration.
• Educate families on disease progression, hospice philosophy, and hands-on caregiving techniques.
• Act as a liaison between physicians, social workers, chaplains, and caregivers to ensure seamless care.
• Participate in on-call rotation, providing after-hours support and emergency visits as needed.

• Licensure:
  Current Registered Nurse (RN) license in the state of Iowa
• Experience:
  
  Hospice or Palliative care experience preferred
• Certification: BLS required; CHPN a plus
• Skills:
  
  Compassionate communication, clinical expertise, autonomy, and strong organizational skills
• Other:
  Valid driver’s license, reliable vehicle, and willingness to travel within a 30-mile radius
