Leaders in Care are supporting a council in South Wales to recruit a 0-25 Transitions Social Worker. You will share caseloads with colleagues, work within a supportive senior management team and benefit from structured supervision.
This statutory role requires a Social Work qualification and registration with Social Care Wales. Duties include needs assessments, transition planning, safeguarding, and coordinating support for young people with short breaks where appropriate.
