Job Summary
The Behavioral Health Intake Coordinator’s primary responsibilities involve receiving incoming behavioral health clients in the emergency department to navigate to the appropriate level of care including inpatient voluntary or involuntary bed placement, outpatient IOP, outpatient partial hospitalization, outpatient individual or group counseling. Promoting the transition of patients, obtaining insurance authorization, conducting bed search, or securing outpatient intake appointments and connecting patients to appropriate community resources.
The BH Intake Coordinator will work closely with colleagues and community-based providers to ensure patients receive access to timely, high-quality care and services. Serves as a patient advocate in obtaining necessary services.
Position Responsibilities
Coordinate resources to support transitions in care from the emergency department to voluntary and involuntary inpatient setting or outpatient services. The BH Intake Coordinator will work with insurance providers to obtain prior authorizations. And work with community BH providers to secure outpatient intake appointments. Meets with patients to assess current community-based behavioral health service, if applicable and/or identify service needs and social support needs and in order to develop an aftercare plan.
Collaborates with community providers and support services to arrange for on-site, virtual, or post-discharge service linkages, including follow up to monitor patient connection.
Provides timely feedback to ED nursing staff and attending medical providers to facilitate a timely and safe discharge plan.
Document all interactions in the notes section of EPIC as appropriate. Perform data entry and complete all data records with concise patient information and appropriate coding to ensure proper tracking of metrics.
