Licensed Practical Nurses-Community Support Services
Under supervision of the Director of Nursing or designee, the Community Support Services LPN provides nursing services and education for the agency’s consumer population enrolled in the Community Support Services (CSS) programs.
Consumers are provided nursing services via flexible nursing staff scheduling to cover morning, day and evening consumer needs. Nursing coverage also includes weekends and holidays providing coverage 4 hours in the morning and four hours in the evening.
In addition, as an employee of SJBHR, Inc., the Community Support Services LPN will assume an active role in improving his/her program outcomes and overall organizational performance through participation in agency quality improvement activities and other management meetings as necessary and appropriate.
Certifications, Licenses/RegistrationsCurrent New Jersey licensure as an LPN is required.
A valid driver’s license is required. Driver’s abstract must meet insurance carrier’s requirements.
Upon hire, must satisfactorily complete the SJBHR orientation and competency based adult mental health rehabilitation program training, including CPR and First Aid Certification.
Education and/ExperienceGraduation from an accredited school of practical nursing is required.
At least two years of licensed nursing experience is required. In addition, mental health experience is preferred.
Essential Duties and Responsibilities- Provides a minimum of 5 hours daily of direct face to face nursing and/or mental health rehabilitation services to consumers according to the consumer’s individual rehabilitation plan, and documents observations, information and services provided. - Provides medication administration and monitoring to consumers enrolled in the program.
- Monitors consumers for side effects of medications and reports any problematic reactions to their supervisor and treating physician.
- Provides consumers education regarding medication and other health related issues.
- Provides family education.
- Functions as a primary nurse for assigned consumers ensuring they remain medically and psychiatrically stable, coordinates care between medical providers, functions as an advocate and attends appointments with consumers when appropriate and teaches consumers how to advocate for themselves.
- Serves as a vital member of the interdisciplinary team with first-hand knowledge of the consumers’ mental health and physical health status, behaviors, and response to individualized rehabilitation plan objectives.
- Provides consumer support, on-site consumer supervision and encourages development of life skills necessary to sustain successful living within the community while offering services appropriate to meet the consumers’ needs.
- Prepares clear, sound, accurate, and informative documentation of all consumer interaction and corresponding follow-up activity in accordance with CSS documentation standards.
- Provides monthly clinical record audits.
- Contacts third‑party payors for prior authorization of medical procedures.
- Reviews labs and medical records. Reports abnormal findings to their supervisor and ordering physician.
