Delaware County, Pennsylvania - Post-Acute Care Physician Opportunity Clinical Role:

- This is a full-time (5 days per week) primary care role
- You will have the opportunity to manage complex patient cases in post-acute settings (skilled nursing and independent living facilities)
- You will work alongside a collaborative interdisciplinary team and lead initiatives that make a tangible difference in patient health and quality of life
- Primary care rounding in skilled nursing facilities
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Collaborate with clinicians, facility leaders and others regarding care and services
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Provide oversight, guidance, and mentorship to our nurse practitioners
- Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of car
- Qualifications
- Minimum of 1 year of experience, with a commitment to delivering high-quality, patient-centered care
- Post-acute care experience is beneficial, though not required, as we provide comprehensive support and development opportunities
- Active and unrestricted Pennsylvania license (or in process)
- Active and unrestricted DEA/prescriptive authority (or in process)
- A deep commitment to improving patient outcomes, leading clinical excellence, and driving change in the healthcare landscape
- Internal medicine (IM), family medicine (FM), emergency medicine (EM), hospitalist or candidates with any related experience are welcome to apply
- Compensation is fee for service (FFS). Expected compensation is an estimated range of $180,000 to $291,000 annually with no cap on productivity income potential.
