Revital Rehab Services is seeking a Full-Time Speech-Language Pathologist (SLP)/CFY to provide exceptional care at Premier at Perry Village, a skilled nursing and rehabilitation center in New Bloomfield, PA. In this role, you’ll play a vital part in helping residents improve their communication, cognitive, and swallowing abilities—enhancing both function and quality of life.
What You’ll Do:Evaluate residents and develop personalized, evidence-based treatment plans
Provide direct therapy services aligned with clinical best practices and facility guidelines
Collaborate with families, caregivers, and interdisciplinary staff to support therapy outcomes
Educate residents and caregivers on strategies to ensure the carryover of skills into daily routines
Maintain accurate documentation and track patient progress toward therapeutic goals
Active Speech-Language Pathology license in the state of Pennsylvania
Strong clinical judgment and communication skills
