• Establish and implement individualized physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.
• Apply evidence‑based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
• Provide physical therapy treatment consistent with patients' needs assessment.
• Perform reassessment when indicated and modify plans accordingly.
• Communicate patient progress and limitations to supervisors, physicians, and other team members.
• Develop measurable goals and time frames for each patient.
• Adhere to departmental billing guidelines and complete all required documentation in accordance with policies and regulatory requirements.

• Bachelor’s Degree in Physical Therapy.
• Ability to work at various locations on a per diem basis.
• Knowledge of best practices in physical therapy and strong clinical reasoning skills.
• Strong patient communication skills and a friendly, supportive manner.
• Ability to travel to patient care assignments in Home Care settings.
• American Heart Association Basic Life Support certification (State of Pennsylvania) upon hire.
• Licensed Physical Therapist in the State of Pennsylvania upon hire.

• Master’s or Doctorate degree in Physical Therapy.
• At least one year of clinical experience in Home Care.

Lift and carry 40–50 lbs. Examples: push/pull patients on bed and stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of patient weight, frequent to continuous standing/walking, 8–10 miles per shift. May require kneeling, bending, squatting, and CPR.
