Position DescriptionThe IBHS Case Manager position is an integral part of Kaleidoscope ABA's mission to deliver therapeutic services to children with autism. The role's primary responsibility is to manage the health and proactive relationship with children, families and school communities served under the Kaleidoscope ABA IBHS program. The Case Manager will maintain a census/caseload that meets the standards set in the program budget and provide care coordination support to children and families in the program.
EssentialFunctions
- Assess the service needs based on a Physician/Psychologist evaluation by completing the IBHS intake.
- Identify, link, and coordinate services including preparation and distribution of IBHS packets and starting up new children.
- Ensure children are CBH eligible to participate in IBHS.
- Facilitate access to services, coordinate with the clinical and recruiting teams.
- Maintain a schedule of key deadlines for each case.
- Work with the clinical team to monitor the effectiveness of services and re‑assess service needs.
- Maintain appropriate documentation of all care coordination/case management services in the electronic health record.
- Meet with the client and responsible parent within the established parameter of initial contact and maintain follow‑up meetings weekly.
- Establish and maintain coordination with CBH, schools, day‑care, providers, or other child‑serving systems to ensure coordination of care.
- Work collaboratively with the clinical treatment team (BCBA, LBS, BHT) to ensure services are coordinated.
- Coordinate weekly service schedules of assigned providers.
- Support the collection and verification of weekly services hours and documentation for appropriate billing.
- Assume other duties assigned by Clinical Supervisor/Lead Clinician and/or Team Leader.
- Bachelor's degree in Social Work, Social Science, Nursing, or a related discipline from an accredited institution.
- Minimum of three (3) years’ case management, counseling, or direct care mental health experience with children and families.
- Knowledge of IBHS regulations is a plus.
- Reliable transportation.
