Job Summary:We are seeking a compassionate and experienced Social Services Director to join our interdisciplinary team at Willow Brook Rehabilitation and Healthcare Center in Kutztown, Pennsylvania. As a Social Services Director, you will play a vital role in promoting and protecting the rights and well‑being of our patients/residents. Responsibilities:- Develop and implement comprehensive care plans for each resident's social services needs
- Hold meetings with patients/residents, families, and significant others as requested, to provide support and information to improve quality of life
- Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers
- Establish relationships and maintain contact and referral flow with community-based agencies and services
- Bachelor's degree from an accredited school of Social Work or related field is required
- At least one year of supervised social work experience in a healthcare setting, working directly with individuals, is preferred
