Claremont Nursing & Rehabilitation Center is seeking a dedicated and compassionate Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. This is an excellent opportunity to work in a collaborative Skilled Nursing and Long-Term Care environment focused on helping residents achieve their highest level of function and independence.
Position Details
Full-Time
In-House Rehabilitation Program
Skilled Nursing Facility (SNF) / Long-Term Care (LTC)
Supportive interdisciplinary team
Consistent caseload
Qualifications
Current Pennsylvania Physical Therapist license or eligibility for licensure
Strong clinical, communication, and documentation skills
Commitment to providing high-quality, patient-centered care
SNF/LTC experience preferred, but not required
Technology
Net Health EMR
Point Click Care (PCC)
Responsibilities
Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence
Collaborate with physicians, nursing staff, and rehabilitation team members
Complete timely and accurate documentation
Ensure compliance with facility, state, and federal regulations
