Concordia Home Health and Hospice of Bethlen is seeking a full-time or part- time
Occupational Therapist OT
to cover Ligonier, Latrobe, Greensburg, and surrounding areas.

The Occupational Therapist [OTR/L] is a registered and licensed qualified professional who directs, supervises, evaluates and provides occupational therapy services to patients in their home or current residence as prescribed by the attending physician.

• Making home visits to patients in the Ligonier Valley and surrounding service area
• Responsible for the initial occupational therapy evaluation and assessment of functional status, endurance, visual coordination, motor skills, environmental hazards or barriers to independent living
• Setting up and implementing the treatment plan, participating in multidisciplinary care in conjunction with the nursing team
• Reporting and recording care provided for both home health and hospice patients under the direction of the patient's physician
• Identifying equipment needs and environmental adaptations
• Educating patient, caregivers and family in rehabilitative care
• Supervising the Certified Occupational Therapist Assistant in implementing the plan of care
• Revising therapy plans as needed to enhance outcome for patients
