Responsibilities- Deliver skilled physical therapy and rehabilitation therapy in patients' homes
- Develop and implement individualized treatment plans.
- Collaborate with an interdisciplinary care team.
- Document patient care using EMR systems (HCHB)
- Licensed Physical Therapist in Pennsylvania
- Recent graduates are encouraged to apply; this role offers opportunities for learning and professional development
- Valid driver's license and reliable transportation
