As an LPN, your primary purpose is to provide direct nursing care to residents and supervise nursing assistants. You will record and communicate resident status, coordinate medication administration, and maintain accurate documentation.

• Direct day‑to‑day staffing of nursing assistants in accordance with long‑term care facility regulations.
• Make written and oral reports and recommendations regarding shift activities.
• Transcribe physician orders into resident charts, medication cards, and treatment plans.
• Chart nursing notes accurately and descriptively.
• Fill out accident/incident reports and submit them to the Director.
• Record new or changed diet orders and forward information to the Dietary Department.
• Prepare and administer medication as ordered, verifying resident identity to prevent errors.
• Maintain accurate narcotic records and notify the Nurse Supervisor of discrepancies.

On‑the‑job training is provided.

• Current, unencumbered, active LPN/LVN license in Pennsylvania.
