Registered Nurse

We are seeking skilled and compassionate Registered Nurses to deliver high-quality, patient-centered care. In this role, you’ll provide vital nursing services directly in patients’ homes, helping them manage conditions, recover safely, and maintain independence.

As a Registered Nurse in home care, you will be responsible for providing high-quality patient care, collaborating with physicians and multidisciplinary team members, and ensuring the smooth operation of the healthcare facility.

• Perform comprehensive in-home nursing assessments and re-assessments
• Develop, implement, and revise individualized care plans in accordance with physician orders
• Provide skilled nursing services including wound care, injections, catheter care, and medication administration
• Monitor patient status and response to treatments, documenting all care and changes in condition
• Educate patients and caregivers on disease management, medication use, and safety practices
• Coordinate with other healthcare professionals including PTs, OTs, HHAs, and social workers
• Ensure compliance with all Article 36 regulations and agency policies
• Participate in quality assurance activities, case conferences, and staff meetings

• Graduate of an accredited nursing program (Associate or Bachelor’s degree)
• Minimum of 1 year of experience in home health, hospital, or clinical setting preferred
• Strong clinical judgment, communication, and patient education skills
• Self-motivated and able to work independently in the field
• Must pass drug screening, and health clearances
• Valid driver’s license and/or reliable transportation
• Bachelor’s degree in Nursing (BSN)
