How you make a differenceThe Chronic Care Nurse (RN) is responsible for managing and providing timely care to patients with chronic medical conditions by working closely with on‑site physicians and mid‑level providers. This position ensures that chronically ill patients are seen on a routine and scheduled basis according to their disease process and tracks and manages the overall care of these patients.
Key Responsibilities- Maintain a comprehensive patient log and schedule for chronic care patients.
- Assess patients as directed by physicians or mid‑level providers and prepare patients for visits, ensuring all required diagnostic testing is complete.
- Provide patient education, address special needs, schedule off‑site consultations, and ensure off‑site recommendations are presented to providers.
- Maintain chronic care statistical data, review aftercare recommendations, conduct medication compliance studies, and maximize use of facility services.
- Ensure compliance with National Commission on Correctional Health Care standards for chronic care programs.
Education
- Must be a graduate of an accredited school of nursing.
Experience
- Three (3) years of relevant clinical experience is required. Prior medical/surgical or emergency/trauma experience is preferred. Previous corrections experience preferred.
Licenses/Certifications
- Currently licensed as a Registered Nurse in the state in which services are provided and possess and maintain current CPR certification.
