This role provides primary care inside a skilled nursing facility under the ISNP program, managing a consistent panel and focusing on keeping residents in place and out of the hospital.

• Monday–Friday, typically 4 days rounding + 1 admin day
• Flexible daytime schedule — structure your day as long as residents are seen and charting is completed
• No nights, no weekends required for daily rounding
• Call availability:
  Monday–Friday, 8 AM–5 PM only (after-hours covered separately)
• Admin day is remote, includes charting time and team meetings
• Mileage reimbursement + full benefits: medical, dental, vision, 401(k) match, HSA, PTO

Base Salary: ~$120,000 + $4,000 annual bonus

Per-visit or additional bonus structures may be discussed as census grows — higher earning potential with established patient panel.

• Provide longitudinal primary care to ~75–80 long-term care residents per month (panel-based)
• Round onsite 4 days per week between two facilities (typically Warren & Marienville)
• One remote admin day weekly
• Manage both chronic and acute conditions
• Focus on “treat-in-place” care models to prevent unnecessary hospitalizations
• Coordinate with facility staff and internal medical leadership as needed

• Board-Certified Physician Assistant licensed in Pennsylvania
• Experience with adult/geriatric populations in primary care, post-acute, or SNF settings preferred
• Comfortable working independently inside facilities
