RN​/LPN Personal Care Part Time

Position Summary

Grace Inspired Living is hiring nurses LPN/RN for a Part‑time night shift 11pm‑7am to provide personal care and clinical services in a supportive residential community for older adults.

• Report at start of shift with pocket care plans/worksheet and receive shift report.
• Assume leadership by coordinating care of residents with appropriate personnel.
• Administer scheduled medications in accordance with facility, state and federal regulations.
• Count narcotics with off‑going nurse and document relevant charting (alert charting, incidents/accidents, vital signs).
• Assist with tube feedings, wound assessments and medication room checks as needed.
• Participate in admission assessments, transfers, deaths, physician orders and physician rounds.
• Respond to any emergencies within Personal Care and assist with cleaning/defrosting medication room refrigerators.
• Complete all other duties as assigned.

• Valid LPN or RN license in the state of Pennsylvania.
• Experience providing personal care and nursing services in a skilled nursing or long‑term care setting.
• Strong communication, documentation, and teamwork skills.
• Ability to work independently and follow established protocols during a 12‑hour shift.
