Crisis Services Nurse (LPN / RN) – Goshen

• Assess individuals for admission using a medical stability approach.
• Assess physical health and perform brief health assessment.
• Deliver care for minor physical health challenges, collect vitals, medical and substance use history.
• Treat minor wounds.
• Administer medication with orders from the center provider.
• Facilitate escalation of care when clinically indicated.
• Help develop and refine center policies and procedures.
• Document all necessary information in patient charts.
• Other duties as assigned.

• Current Indiana LPN or RN license.
• At least two years of nursing experience.
