As a LPN /
RN Nurse, you will play a pivotal role in ensuring the physical and psychosocial needs of our cherished residents are met with the utmost dignity and compassion. Your dedication and expertise will be the driving force behind the positive transformation you bring to their lives.

• Start your shift by receiving a comprehensive shift report to ensure seamless care continuity.
• Assume leadership by coordinating the care of residents with appropriate personnel.
• Maintain accountability by accurately counting narcotics with the off‑going nurse.
• Adhere to facility, state, and federal regulations while administering scheduled medications, performing scheduled treatments, wound assessments, and documentation as per facility policy.
• Document vital information on residents, including 7‑day assessments for MDS documentation, alert charting, monthly summaries, and incidents/accidents.
• Participate in admission assessments, transfers, deaths, physician orders, and prepare for physician rounds.
• Maintain regular attendance, which is essential to the team's success.
• Communicate resident or staff issues to appropriate personnel.
• Attend meetings and in‑services as required.
• For the 11 pm–7 am shift, nurse the neighborhood medication sheet re‑caps.

Education

• Completion of a Practical Nurse Program.
• Completion of a Registered Nurse program.

Experience

• One year of experience in a nursing environment.
• Preferred:
  Three years of experience in geriatric nursing.

Professional Membership, Licensures, Certifications

• Licensed Practical Nurse in the state of PA.
• Registered Nurse in the state of PA.
