The Admission RN is responsible for the entry of patients and families into the spectrum of services provided by Penn Medicine Hospice. As a member of the Interdisciplinary Team (IDT), the Admission RN provides direct care to adult patients and demonstrates safe practice within the domains of clinical judgment and decision-making, nurse‑patient/family relationship, clinical leadership, and clinical scholarship. The Admission RN works closely with others on the healthcare team to facilitate the coordination of care across the continuum.
The individual reviews documentation of potential clients who are referred to hospice, arranges for initial contact in a timely fashion, and communicates plans for contact with client, family and referral sources. In addition, the Admission RN provides information to potential hospice patients, assists with the decision‑making process, performs assessments that assure the appropriate level of care is delivered, and expedites entry into service delivery systems.
The Admission RN attends to the values and rituals important to the patient‑family‑community environments while upholding nursing practice and safety standards. The Admission RN instructs the primary caregiver of immediate care needed and informs primary nurse of the initiation of the plan of care. The Admission RN is responsible for practicing within the hospice regulations and reimbursement criteria, determining the skilled nursing care and visit pattern to maintain quality and meet the patient and family’s goals.
The Admission RN functions within the interdisciplinary team to facilitate communication, promote, and support symptom and disease management.
