• Conduct 25-30 patient visits per week, including a mix of start-of-care and revisits
• Perform and document OASIS assessments for eligible visits
• Utilize Epic for charting, care planning, and documentation
• Assess patient needs, develop and update individualized care plans, and coordinate with the interdisciplinary team
• Provide patient and caregiver education, promote safety, identify and resolve potential issues, and ensure timely follow-up
• Communicate clearly with patients, families, and supervisors to maintain quality care and accurate records

• Minimum 2+ years of recent and relevant home health nursing experience
• Proficiency with OASIS documentation and Epic electronic medical records
• Ability to manage 25-30 visits per week with a mix of start-of-care and revisits
• Daytime schedule availability and willingness to travel within the service area
• Commitment to a 12-week travel assignment with guaranteed hours
