Deliver essential care in a dynamic per diem opportunity focused on home health risk assessments. As a Nurse Practitioner providing in-home services, youll engage directly with Medicare Advantage plan members for comprehensive assessments throughout Luzerne County. Your schedule is highly flexible, with shifts available between Monday and Sunday, allowing you to build a work pattern that fits your lifestyle. Assignments may include visits for annual health assessments or post-discharge check-ins, and each evaluation typically takes about 45 minutes.
You will have the freedom to set your own minimum weekly or bi-weekly hourly commitments.

- Conduct thorough annual home health risk assessments in patient homes
- Review and evaluate previous and current medical history, existing conditions, and medication lists
- Educate patients regarding their medical conditions and medication management
- Identify gaps in care and make actionable recommendations to enhance patient outcomes
- No requirement to develop treatment plans or prescribe medications
- Active Nurse Practitioner license in Pennsylvania
- Current Basic Life Support (CPR) certification
- Board Certification through AANP or ANCC
- Minimum of six months experience as an NP preferred
- Master of Science in Nursing (MSN) from an accredited program
