This role supports individuals with intellectual or developmental disabilities and Autism by linking them to helpful services and natural supports. As a County Caseworker 2, you will perform the following duties:

• Support Planning:
  Develop and update Individual Support Plans with individuals and their teams
• Service Coordination:
  Coordinate needed services based on individual needs and goals
• Community Guidance:
  Encourage individuals to explore community work opportunities when appropriate
• Training Participation:
  Complete initial Supports Coordinator training and ongoing learning activities
• Field Visits:
  Travel to meet with individuals and teams to review supports and progress
• Documentation Work:
  Enter required data and maintain organized records

The Departments under the Wayne County Human Services Department of Wayne County Government are Civil Service covered positions through the Pennsylvania State Civil Service Commission. Please note these are County positions and carry county salaries and benefits package.
