Piper Companies is seeking a Supports Coordinator to join a mission-driven human services organization located in Montgomery County, PA. to support individuals with Intellectual Disabilities () and Autism Spectrum Disorder (ASD). The Supports Coordinator role is ideal for a compassionate, organized professional who is passionate about advocacy, service coordination, documentation, and helping individuals access services.
Responsibilities of the Supports Coordinator:- Manage a caseload of individuals with ASD
- Coordinate, monitor, and advocate for services that support health, safety, and quality of life
- Conduct monitoring visits in homes, community settings, and day programs throughout Chester, Montgomery, and Delaware Counties
- Connect individuals with services including transportation, residential supports, job coaching, and community programs
- Maintain communication with individuals, families, and providers to ensure effective service delivery
- Complete detailed documentation including case notes, monitoring reports, billing records, and compliance documentation
- Bachelor's degree required
- Degree in Psychology, Social Work, Criminal Justice, Human Services, or related field preferred
- Valid driver's license, reliable personal vehicle, and automobile insurance required
- Ability to travel locally throughout assigned territory
- Strong written communication, organizational, and time management skills
- Passion for supporting individuals with disabilities and willingness to learn through extensive training
- Salary range: $42,/year
- Mileage reimbursement for business travel
