Delco Pa is seeking a County Caseworker 2 to locate, coordinate, and monitor essential services that support participants in daily life. You will help individuals access community resources and ensure services align with their Individual Support Plan.
Responsibilities include assisting participants in finding and arranging services, coordinating ISP development with participants, families, and providers, and maintaining regular contact to verify delivery of supports.
