Job Responsibilities
- Admissions, Discharge, and Transfers of residents
- Medication requisitions with pharmacy and physicians
- Medication administration
- Following infection control, emergencies, and other center policies
- Collaborating with resident’s interdisciplinary team to provide the best outcomes
- Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
- Work under direct supervision in accordance with the state‑specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment
- Deliver nursing care to patients/residents requiring long-term or rehabilitative care
- Collect patient/resident data, make observations, and report pertinent information related to the care of the patient/resident
- According to state‑specific regulations, implement the patient/resident plan of care and evaluate the patient/resident response
- In accordance with state‑specific regulations, direct and supervise caregiving by other nursing personnel in selected situations
- Maintain knowledge of necessary documentation requirements
- Maintain knowledge of equipment set‑up, maintenance and use (i.e., monitors, infusion devices, drain devices, etc.)
- Maintain confidentiality and patient/resident rights regarding all patient/resident/personnel information
- LPN (Licensed Practical Nurse) licensed by the State
- CPR Certification required
- Two Years prior nursing experience preferred
