Licensed Practical Nurse (LPN)

Job Responsibilities
  • Admissions, Discharge, and Transfers of residents
  • Medication requisitions with pharmacy and physicians
  • Medication administration
  • Following infection control, emergencies, and other center policies
  • Collaborating with resident’s interdisciplinary team to provide the best outcomes
  • Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
  • Work under direct supervision in accordance with the state‑specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment
  • Deliver nursing care to patients/residents requiring long-term or rehabilitative care
  • Collect patient/resident data, make observations, and report pertinent information related to the care of the patient/resident
  • According to state‑specific regulations, implement the patient/resident plan of care and evaluate the patient/resident response
  • In accordance with state‑specific regulations, direct and supervise caregiving by other nursing personnel in selected situations
  • Maintain knowledge of necessary documentation requirements
  • Maintain knowledge of equipment set‑up, maintenance and use (i.e., monitors, infusion devices, drain devices, etc.)
  • Maintain confidentiality and patient/resident rights regarding all patient/resident/personnel information
Licensed Practical Nurse (LPN) QUALIFICATIONS
  • LPN (Licensed Practical Nurse) licensed by the State
  • CPR Certification required
  • Two Years prior nursing experience preferred
