Long-Term Acute Care(LTAC) Registered Nurse (RN)
The LTAC RN is a dedicated and skilled nursing professional responsible for providing comprehensive care to medically complex patients requiring extended hospitalization and specialized treatment. This role demands strong clinical judgment, adaptability, and the ability to manage multiple priorities in a dynamic environment. The RN collaborates with physicians, interdisciplinary teams, and support staff to ensure safe, effective, and patient-centered care focused on long-term recovery.
Essential Duties and Responsibilities- Apply the nursing process to assess, plan, implement, and evaluate care for patients with chronic, critical, and post-acute conditions.
- Conduct thorough patient assessments, including vital signs, physical exams, health histories, and monitoring of complex clinical parameters.
- Manage advanced therapies such as ventilator support, tracheostomy care, and complex wound care.
- Administer medications, treatments, and IV therapies as prescribed.
- Monitor and document patient responses to interventions and changes in condition.
- Collaborate with healthcare providers to develop and adjust individualized care plans focused on long-term goals.
- Prepare patients for diagnostic tests, procedures, and transitions of care.
- Provide education to patients and families on disease processes, medications, discharge planning, and self-care strategies.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Adhere to infection control, safety, and quality standards.
- Respond promptly to emergencies such as rapid deterioration, respiratory distress, or code situations.
- Advocate for patients and uphold patient rights and confidentiality.
- Current Registered Nurse License in the state of practice.
- Minimum one year of professional nursing experience; LTAC, ICU, or Med-Surg experience preferred.
- Completion of Amergis' Competency Program (if applicable).
- Current BLS certification (ACLS preferred).
- Proficiency in ventilator management, tracheostomy care, and complex wound care.
- TB screening and health clearance (as required by contract or regulation).
- Compliance with all federal, state, and local regulations.
- Successful completion of new hire orientation and LTAC-specific training.
- Understanding of patient confidentiality and HIPAA regulations.
- Strong communication skills and proficiency in English.
- Basic computer literacy.
- Must be at least 18 years of age.
