Schedule:
On‑call duty, nights, weekends, and holidays as assigned.
- Understand and adhere to policies and procedures.
- Implement nursing care plans for patients.
- Provide nursing services, treatments, preventive procedures and diagnostics.
- Initiate preventive and rehabilitative nursing procedures.
- Observe signs/symptoms and report to physician and RN; monitor medication and response.
- Teach and counsel patients and significant others on care needs.
- Collaborate with RNs to evaluate effectiveness of LPN services.
- Maintain accurate records of observations, treatments, and care.
- Participate in medical record audits.
- Attend staff meetings, patient care conferences, and in‑services.
- Submit schedule changes to Director of Clinical Services/Nursing Supervisor daily.
- Participate in patient care conferences with multidisciplinary team.
- Prepare clinical and progress notes.
- Assist physicians and RNs with specialized procedures.
- Prepare equipment and materials for treatments.
- Assist patients in learning self‑care techniques.
- Graduate of a state‑approved school of practical (vocational) nursing and licensed in the state(s) of practice.
- Minimum of one year of nursing experience; home‑health experience highly preferred.
- Acceptance of agency philosophy and goals.
- Ability to exercise initiative and independent judgment.
- Must hold BLS certification; valid LPN license.
Work occurs in agency offices and patient homes, traveling to patient homes as necessary.
Job RelationshipsSupervised by Director of Clinical Services, Nursing Supervisor, or RNs.
Risk ExposureHigh risk.
Lifting RequirementsAbility to perform physical activities including heavy lifting, bending, and standing for extended periods.
