You will be responsible for undertaking person‑centred, strength‑based assessments to enable people who have long‑term physical health or social care needs to maximise their functional and occupational performance in order to be able to continue to live safely and independently within their local communities.

As part of this, you will be involved in focusing upon preventing, reducing and delaying the need for care and support. This, in turn, will help to ensure that our residents, families, and communities have the right support in the right place at the right time.

You’ll be involved in completing environmental and functional assessments within a person’s home, linking in with reablement and rehabilitation services within your Primary Care Network area as and when needed; assisting with the provision of information and advice; providing equipment; recommending minor and major adaptations and/or rehousing advice, in order to promote the person’s wellbeing and independence.

Your role will also involve seeking new, creative, innovative, cost‑efficient and effective solutions to the person’s situation. You may also be involved in seating and/or moving and handling assessments, advising on optimal care where appropriate.

You’ll need to be innovative with strong interpersonal skills and have the ability to work well under pressure and an understanding of the six principles of Safeguarding. You will be expected to use evidence‑based practice to deliver positive outcomes.
