Position Summary
Assess, plan, and deliver individualized rehabilitation programs that improve mobility, relieve pain, increase strength, and restore function following illness or injury.
What You’ll Do
Perform and document initial evaluations to identify functional limitations and establish appropriate treatment plans.
Develop and implement individualized therapy programs to improve physical function, promote independence, and prevent further decline.
Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.
Educate patients and families on exercises, safe mobility, and strategies to support recovery.
Collaborate with team members and other healthcare professionals to ensure continuity of care.
Follow infection control practices and universal precautions during patient care.
Complete accurate, timely documentation within company guidelines.
Support agency philosophy and administrative policies.
Qualifications
Graduate of an accredited physical therapy program with successful completion of the state licensure examination.
Current, valid Physical Therapy license.
One year of home health experience preferred.
Current CPR certification.
Valid driver’s license and auto insurance.
Home Care Home Base experience preferred.
Physical Requirements
Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Ability to lift, carry, pull, or push up to 100 pounds.
