Synergy Care is looking for a qualified Licensed Physical Therapist for an open position at an Independent Living Community in Peoria, IL. We are looking for someone who will provide quality patient care and be a team player.

• Provides physical therapy and related services for patients, caregivers and facilities in accordance with the principles and practices of occupational therapy and with Synergy Management Solutions guidelines.
• Provides quality PT including: evaluation of patients, development of initial and ongoing treatment & discharge plan, supervision of assistants and aides and participates in case management.
• Represents Synergy Management Solutions’ commitment to quality & service in all interactions with facility.
• Adheres to all state regulations regarding occupational therapy.

In order to be considered for this position you must be a graduate of an accredited school for Physical Therapy or will graduate from such an institution within the next six (6) months.
