Job DescriptionRegistered Nurse – Medical Case Manager. Contract to Hire Opportunity
As a Medical Case Manager RN, you will receive children taken into DCFS custody within 24 hours. You will act as the health works lead agent, requesting all medical records and monitoring any medical conditions, including immunizations, dental, and vision care. This is a Monday–Friday, first shift position, with no weekends, working with DCFS caseloads, families, and children. You will maintain complete documentation and address foster parents' questions.
You will manage a high-risk caseload in our Peoria office.
Ideal candidates will demonstrate organization, excellent charting skills, and a caring disposition. Our client is a nonprofit attached to Human Services, providing medical assistance to the community. - Provide case management services to DCFS children.
- Support the needs of children and families and plan services.
- Monitor family progress towards goals in case files.
- RN with a 2-year degree.
- Experience working with children and families.
- Valid driver’s license and access to a car.
- Pass background check.
