Position: Nurse Practitioner (Skilled Nursing)

Nurse Practitioner – Skilled Nursing Facility (SNF) – Perry, Florida

Base pay range: $/yr - $/yr

Major Responsibilities:

  • Act as a skilled nursing facility’s leading provider of internal medicine services.
  • Collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while overseeing the care of all residents.
  • Oversee day-to-day care for short- and long-term residents with assistance from nursing staff and guidance from medical directors and attending physicians.
  • Evaluate new admission visits and coordinate discharge visits.
  • Conduct monthly stability visits for long-term residents and evaluations for patients exhibiting changes in condition.
  • Provide advanced care planning, comprehensive health assessments, differential diagnosis, and prescribe treatments under protocol for acute and chronic illnesses.
  • Prescribe all necessary medications and treatments.
  • Perform comprehensive physical examinations of patients.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and specialists regarding patient care.

Qualifications:

  • Advanced Practice Licensure (APN) with a Florida license.
  • Active license and DEA number required.
  • Previous experience in nursing or other medical fields.
  • Ability to build rapport with patients and their loved ones.
  • Ability to thrive in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Master’s degree required.
