Make Evenings Count Looking for a nursing role where you’re a leader, not just another body on the schedule? Three Meadows Post Acute in Perrysburg is hiring a 2nd shift LPN to join a supportive team that puts residents first. On second shift, you set the tone for the evening: guiding your CNAs, caring for residents, and keeping the unit running smoothly.

What You’ll DoProvide hands-on nursing care to residents, including medication administration, treatments, wound care, and monitoring changes in condition

Lead your unit’s evening shift by supervising and supporting nursing assistants and making daily rounds

Be the link between residents, families, and physicians, including physician orders, family updates, and care coordination

Keep care plans on track, working with the care plan team and contributing to quality assurance

Help new residents settle in, from admission and orientation to discharge planning

Promote a safe, infection-free environment and help staff follow safety and infection control practices

What You’ll Bring A graduate of an approved LPN program

Current, unencumbered Ohio LPN license in good standing with the State Board of Nursing Current CPR certification

Leadership skills and a team-first attitude

Patience, tact, and compassion for working with older adults and their families

Ability to lift 50 lbs and move through the shift comfortablyPoint Click Care  (PCC) experience is a plus, but not required
