Full-time - Schedule:
Monday–Friday, 8am–5pm - Salary: $22–$25 per hour
- Bilingual requirement:
Spanish
- Perform community informational service events.
- Deliver and pick up orders, progress notes to physicians, medical forms/records from physician offices and facilities; explain documents and facilitate patient care communications; obtain required signatures.
- Implement provider relations strategies to positively affect referral base and revenue in the assigned territory.
- Identify and visit case managers, discharge planners, social workers, physicians, and other potential referral sources on a regular basis.
- Maintain knowledge of industry competitors to help Elara Caring retain and grow market share.
- Collaborate with Leadership to plan and participate in trade shows and other opportunities to promote Elara Caring in the community for referral generation or field staff recruitment.
- Collaborate with Leadership in contract assessment, preparation, and negotiation as needed.
- Serve as educational resource to potential referral sources regarding the range of home health care services available.
- Market Elara Caring services by maintaining active membership in local networking groups and attending meetings and events.
- Receive referrals from physicians, hospitals, and other agencies and coordinate with internal departments to support patient admission.
- Serve as a liaison between the agency, physicians, facilities, referral sources, and hospitals in the community.
- Promote Elara Caring’s mission and policies to ensure quality of care.
- Maintain patient and staff privacy and confidentiality in accordance with HIPAA Privacy standards.
- Performs other duties and projects as assigned.
- Experience in the Home Health industry is preferred
- Strong verbal and written communication skills
- Effective organizational and interaction skills
- Knowledge of healthcare industry is preferred
- Flexibility, enthusiasm, and willingness to cooperate with others
- Understanding of how referral sources make decisions and who the decision makers are
- Commitment to professional growth and competency
- Must have a dependable vehicle, valid driver’s license, and current auto insurance as required by state law
