Salary: $45,000 per year

• Provide case management services to individuals residing in the Ozanam Inn Shelter and Permanent Supported Housing.
• Screen, assess, plan services, link, intervene in crisis, and de‑escalate situations.
• Manage relationships with a broad range of community providers and teach life skills.
• Transport individuals to housing opportunities, assist clients in applying for housing, and build connections with other agencies.
• Utilize HMIS case management software.

• Bachelor’s degree (B.A.) from an accredited college or university, preferably in Social Work or a similar Human Service field.
• At least two (2) years of relevant work experience and demonstrated ability to provide effective services to clients.
• Experience with conflict resolution and mediation.
• Bilingual in Spanish is a plus.
• Must have a valid driver’s license and a clean driving record.
