Physician-led, team-based model focused on coordinated, patient-centered care Opportunity to provide care across the continuum in a value-based care setting Procedures may include, but are not limited to: simple skin surgery, fracture care, joint injections, maternity care, IUD placement Full-time schedule options: 4 or 4.5 workdays per week Open to board certified or board eligible Internal Medicine physicians Must be comfortable working alongside and supervising advanced practice providers

Qualifications:
Unrestricted medical license in Indiana or ability to obtain Board certified or board eligible in Internal Medicine or Family Medicine Active and unrestricted DEA license or ability to obtain before start
