• Assessment and implementation of SLP interventions (i.e., swallowing assessments, dysphagia management, communication, etc.) in a home‑based setting
• Use of a collaborative and interdisciplinary team‑based approach to care and services
• Support a holistic and restorative approach to care (i.e., enhanced stroke pathway; cognitive communication supports, etc.)
• Leverage digital health platforms to support care delivery, collaboration, documentation, and intervention

• Registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
• Valid driver’s license and vehicle insurance
• Focus on person- and family-centered care
• Comfortable delivering care both in person and using virtual methods/technology
• Passionate, empathetic, good judgement and critical thinking in client decisions
• Excellent communication skills and ability to work in collaboration within an interdisciplinary team and independently
• Flexible with demonstrated critical thinking skills that can be applied to various situations
• Value the role a therapist plays in keeping clients at home where they want to be

The salary range for this position is $153.87 – $168.12 per visit.
