Integrated Care Occupational Therapist

As part of our Integrated Care Program, you’ll collaborate with treatment teams to facilitate seamless transitions from hospital care to home, prioritizing holistic health support and client empowerment.

Key Responsibilities:

• Conduct assessments and provide OT interventions

• Collaborate with interdisciplinary care providers

• Advocate for client independence and self-management

• Use digital health platforms for documentation

Requirements:

• Registered member with the College of Occupational Therapists of Ontario

• Valid driver’s license for community travel

• Excellent communication and judgment skills

• Passionate about family-centered care

• Experience in community-based home care preferred
