* WHAT YOU’LL DO:

** Peterborough Regional Health Centre is pleased to partner with various Peterborough agencies, to offer services through the new Peterborough Homelessness Addiction and Recovery Team (HART) Hub. The Peterborough HART Hub is designed as a low-barrier, trauma-informed, culturally responsive, health care model that integrates primary care, mental health, substance use treatment, housing with supports, and health systems navigation. The Registered Nurse (RN) would be responsible for delivering services effectively and in a way that meets the needs of the individuals they serve.

The Occupational Therapist (OT) will work with clients presenting with significant mental health and/or substance use concerns using a client-centred, evidence-based service model based on standards of best practice.

The OT will collaboratively provide a continuum of client-centred assessment, treatment, rehabilitation, recovery interventions, and case management services for individuals in the community with mental health and addiction challenges and are experiencing homelessness. The purpose being to promote recovery, prevent relapse and to enable clients to improve their quality of life. The Occupational Therapist is an integrated role on the health care team at the Peterborough Homeless and Addiction Recovery Treatment (HART) Hub.
