Impact community health as a Registered Nurse Practitioner at the Peterborough Family Health Team. This permanent position (0.6 FTE) involves health assessments, chronic illness treatment, and collaborative multidisciplinary care.
As part of this role, you will work closely with physicians and health professionals, utilizing your full scope of practice to provide optimal patient care. You will be responsible for diagnosing, treating, and managing health conditions, fostering wellness, and driving health promotion initiatives.
Key Responsibilities:
• Complete comprehensive health assessments
• Treat and monitor chronic health conditions
• Order and interpret diagnostic imaging tests
• Collaborate with physicians for consultations
• Gather and document health statistics
Requirements:
• Current RN registration in Ontario (Extended Class)
• Nurse Practitioner program completion
• 3 to 5 years of community health nursing experience
