Registered Nurse Family Health Team

Deliver patient-centered care as a Registered Nurse with the Peterborough Family Health Team. This permanent role emphasizes outreach and community health services in a team-oriented environment.

In this 1.0 FTE position, you'll collaborate with the clinical team to provide comprehensive nursing services, including care for unhoused and underserved populations. Leverage your clinical decision-making and assessment skills while maintaining proper documentation and administering necessary treatments. You'll play a vital role in promoting health education and navigating patients to appropriate care resources.

Key Responsibilities:

• Execute patient assessments for tailored interventions

• Administer medications and treatments as per directives

• Support chronic disease management and educational initiatives

• Manage lab results and follow-up communications

• Operate RN-led clinics for specific health needs

Requirements:

• Bachelor of Science in Nursing essential

• Registered and in good standing with CNO

• Five years of community or primary care experience

• Active knowledge of CNO standards

• Skill in managing patient relationships
