Position: Registered Nurse – 37.5 hours/week – Permanent )

Overview

Reporting to a PFHT Clinical Manager, in collaboration with a designated lead of the PSM team, the RN will support the delivery of accessible, community-based care to individuals served by the PSM team. They will work within their full scope of practice as outlined by the College of Nurses of Ontario (CNO). This is a 1.0 (37.5 hours per week), permanent position working within the City of Peterborough.

Responsibilities
  • Assess patients to identify appropriate nursing interventions in a low-barrier, outreach-based care environment or in shelter-based clinics
  • Provide comprehensive nursing care including foot care, wound care, and harm‑reduction–informed interventions
  • Initiate and maintain EMR documentation, including patient history, presenting concerns, and care provided
  • Perform phlebotomy (blood draws), administer injections, and support diagnostic testing as required
  • Support the development of appropriate medical directives in collaboration with PSM physicians
  • Administer medications and treatments under medical directives and established protocols
  • Operate RN‑led clinics (e.g., blood pressure management, HPV screening, foot care, wound care, and forms completion)
  • Provide case management and system navigation support, including connecting patients to primary care, specialists, and community resources
  • Conduct outreach in collaboration with the PSM team, including engagement with underserved and unhoused populations using a harm‑reduction approach
  • Support chronic disease management (e.g., diabetes, hypertension) through monitoring, education, and follow‑up
  • Manage and monitor inbox and lab results in collaboration with supporting physicians, ensuring timely follow‑up and continuity of care
  • Administer and record immunizations and provide patient education on indications and side effects
  • Maintain accurate, concise, and confidential EMR documentation
  • Collaborate with an interdisciplinary team to plan, implement, and evaluate patient care in partnership with patients and their families
  • Maintain clinic supplies, medical inventory, and vaccine cold chain (if applicable to program operations)
  • Ensure readiness of emergency equipment and supplies
  • Support and participate in outreach clinics and flexible service delivery models
  • Other related duties may be assigned in support of program operations and patient care needs

Other related duties may be assigned in the interest of the efficient operations of Peterborough’s Family Health Team and the effective management of our patient populations.

Qualifications
  • Completion of a Bachelor of Science in Nursing
  • Minimum five years of clinical experience, preferably in community, outreach, or primary care settings
  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Active Knowledge of CNO Standards & Guidelines
  • Member of Registered Nurses Association of Ontario and active knowledge of best practice guidelines
Skills and Abilities
  • Strong patient assessment, triage, and clinical decision‑making skills in non‑traditional settings
  • Demonstrated ability to work as a self‑directed, autonomous practitioner
  • Experience working under medical directives and willingness to expand scope of practice
  • Commitment to working to full RN scope, with openness to ongoing skill development
  • Experience with phlebotomy, injections, foot care, and wound care considered an asset
  • Knowledge of and experience with Electronic Medical Records (EMRs)
  • Robust communication and interpersonal skills, with the ability to build trust with marginalized populations
  • Experience or strong interest in working with unhoused, underserved, and Indigenous populations, using a trauma‑informed and harm‑reduction lens
  • Knowledge of and comfort providing sexual and reproductive health care, with a commitment to patient choice, autonomy, and non‑judgmental care
  • Ability to work collaboratively within an interdisciplinary team while maintaining independence in clinical practice
  • Strong organizational skills, including managing labs, inbox, and follow‑up care
  • Flexibility and adaptability to meet the needs of a dynamic, outreach‑based program
  • RN prescribing (if applicable/available) considered an asset
