In this pivotal role, you will work closely with the Nurse Specialized in Wound, Ostomy, and Continence (NSWOC) and engage in holistic assessments. Your contributions will include formulating treatment plans, delivering education, and collaborating with an inter professional team to enhance client care. Key Responsibilities:
• Conduct assessments for clients with wound care needs
• Create and adjust care plans based on client evaluation
• Mentor and educate healthcare providers within the organization
• Coordinate with various teams to ensure comprehensive care
• Participate in quality health care monitoring initiatives
Requirements:
• Registration with the College of Nurses of Ontario
• Successful completion of the SWAN program
• Excellent interpersonal skills for effective communication
• Ability to work varied shifts and travel as needed
• Updated immunization and criminal background check required
