Visiting Nurse in Kawartha Lakes

We are looking for compassionate and motivated individuals who can provide support to ALC clients. As a Visiting Nurse, you will perform assessments, develop individualized care plans, and implement best practices to ensure that clients achieve their health goals. Collaboration with care teams is essential for a holistic approach to care.

Key Responsibilities:

• Lead and coordinate care for assigned clients in their homes

• Administer medication and monitor client responses

• Conduct health assessments and document findings

• Facilitate care transitions and connect clients to community services

• Uphold safety and infection control measures at all times

Requirements:

• Registered Nurse in good standing with CNO

• Current certification in CPR and First Aid

• Valid driver’s license and reliable vehicle

• Experience in home health care preferred

• Ability to communicate effectively in English
