Nurse Practitioner FHT

Join the Peterborough Family Health Team as a Nurse Practitioner focused on patient-centered care. This permanent role involves providing comprehensive health assessments and managing chronic illnesses in a multidisciplinary environment.

As an integral member of our team, you will work along with family physicians, nurses, and other healthcare professionals to enhance patient quality of life. This position requires you to operate within your full scope of practice as defined by the College of Nurses of Ontario. Your expertise in health assessments and chronic illness management will be crucial for improving access to primary care in the local community.

Key Responsibilities:

• Conduct thorough health assessments and physical examinations

• Diagnose and treat minor illnesses and injuries

• Monitor and manage chronic illnesses with effective interventions

• Prescribe medications as necessary under your scope of practice

• Collaborate with physicians and team members effectively

Requirements:

• Current RN registration in Extended Class

• Successful completion of Nurse Practitioner program

• 3-5 years of community health nursing experience

• Ability to work flexible hours, including evenings and weekends

• Commitment to patient-centered care and teamwork
